Yizkor Memorial Book 5778

Yizkor, a special memorial prayer for the departed, is recited in the synagogue four times a year, following the Torah reading on the Yom Kippur, on the second day of Sukkot, on Shemini Atzeret and the last day of Passover. Yizkor, in Hebrew, means "Remember." It is not only the first word of the prayer, it also represents its overall theme. In this prayer, we implore God to remember the souls of our relatives and friends that have passed on.

Park Slope Jewish Center publishes an annual Memorial Yizkor Book. The book will be used at our Yizkor Memorial Services on Yom Kippur.
This is an opportunity to commemorate your departed loved ones by having their names published in this book. 

Please fill in the requested information for each person you would like to memorialize. You may use the reverse side of the page if you need more space. The book will be published before Rosh Hashanah, so please return this form no later than September 15, 2017.

It is the custom at PSJC that participants in the Yizkor Memorial Book, contribute at least $18 per loved one listed.

On the next page you will see payment options. You can either:
1. Pay via credit card using our secure credit card processor Authorize.net.
or
2. Choose to add the cost to your account. To do this you must be logged in to your account and choose the pay later option via the drop down menu.

If you choose the pay later you must send in a check payable to: Park Slope Jewish Center at 1320 8th Avenue, Brooklyn, NY 11215.
